An outpatient program that provides individualized treatment for older adults who suffer from emotional, behavioral or mental health disorders.

This program provides intensive outpatient treatment Monday through Friday and patients are able to return to their homes each evening.
The outpatient program assists seniors struggling with:
  • Grief and loss
  • Changes/loss of health
  • Medication education
  • Anxiety, nervousness, worry
  • Relationships
  • Depression
  • Aging/failing memory
  • Anger management
  • Relaxation skills
  • Change in eating/sleeping patterns
  • Low self esteem

The Program
The Senior Transitions program is a medically supervised program tailored to meet the individual needs of older adult patients requiring behavioral health support. Most therapy sessions are in the group format, although individual therapy is provided if clinically indicated.

These groups are facilitated by a trained Masters Level Clinician, under the supervision of a board certified Psychiatrist.
Additionally, the patient’s care is coordinated with families, caregivers and physicians to provide the best treatment solutions for each individual.

Assessments and Referrals
The clinical staff at Senior Transitions is pleased to offer a free confidential assessment for the purpose of determining the need for behavioral health services. Referrals to Senior Transitions may be made by physicians, nurses, social workers, self or others concerned.
